我作为（单位名称）的法定代表人（实 际控制人）和安全生产第一责任人，对本单位的安全生产工作负全面责任，为做好安全生产工作，有效防范和遏制事故发生，现郑重承诺如下：
认真贯彻执行国家、省、市、区关于安全生产的法律、法规、政策和工作要求，积极落实安全生产主体责任，加强基础建设，提升企业安全生产本质水平，努力做好企业的安全生产工作，减少和杜绝安全生产事故，创造良好的安全环境。
一、建立健全以安全生产责任制、安全生产管理制度、岗位操作规程和应急救援预案为主要内容的“三项制度”，并严格执行和落实；
二、认真贯彻落实上级有关部门各项安全生产工作部署；
三、认真开展企业内部“三级安全教育”，主动参加相关部门组织开展的安全生产培训，确保从业人员依法接受“三级安全教育”和安全知识培训，做到相关人员按规定持证上岗；
四、认真开展企业安全生产隐患排查治理活动，建立企业内部安全隐患自查自纠自报工作机制，及时发现和治理事故隐患；
五、依法制定事故应急救援预案，配备必要的救援设备和器材，并定期进行演练，落实岗位应急措施；
六、按要求上报生产安全事故，杜绝各类瞒报、迟报等违法行为，妥善处理伤亡人员、依法赔偿等事故善后工作；
七、保护从业人员在安全生产方面应有的权益，依法参加工伤社会保险，为从业人员足额交纳保险费；
八、依法建立安全生产管理机构，配备符合法定人数的安全生产管理人员，并保证全面履行职责；
九、依法为从业人员提供良好的安全生产条件，强化安全防护措施，在危险部位设臵安全警示标识，为从业人员提供符合国家标准或行业标准的劳动防护用品，同时对各种设施、设备按规定定期进行安全检测、检验，确保安全运行；
十、认真制定作业场所职业危害控制措施，进一步建章立制，按规定开展作业场所职业危害申报和职业卫生安全许可工作，努力改善作业环境，保护从业人员健康利益；
十一、保证新建、改建、扩建工程项目严格履行安全设施“三同时”制度，使安全设施符合国家相关规定和要求；
十二、落实重大危险源管理、监控责任，对重大危险源实施有效监测、监控；
十三、确保资金投入，依法提取安全生产费用，积极采用先进的安全升级技术、工艺，逐步淘汰落后的设施、设备，具备法律、法规、规章、国家标准和行业标准规定的安全生产条件；
十四、履行法律法规规定的其他安全生产职责。承诺期内，若违反有关规定或不落实上述承诺内容，愿意接受法律法规的处罚；导致发生生产安全事故，按下列条款规定接受处罚：
1.法定代表人（实际控制人）：
（1）一般事故接受上一年年收入的 30%罚款。
（2）较大事故接受上一年年收入的 40%罚款。
（3）重大事故接受上一年年收入的 60%罚款。
（4）特别重大事故接受上一年年收入的 80%罚款。
（5）发生重大及以上事故的，接受终身不再担任本行业企业主要领导职务的处罚。
（6）同时接受相应的党纪、政纪处分，构成犯罪时，接受相应的刑事处罚。2.企业：
（1）一般事故接受 20 万元以上 50 万元以下罚款。
（2）较大事故接受 50 万元以上 100 万元以下罚款。
（3）重大事故接受 100 万元以上 500 万元以下的罚款。
（4）特别重大事故接受 500 万元以上 1000 万元以下罚款；情节特别严重的，接受 1000 万元以上 2000 万元以下的罚款。
（5）同时接受有关部门做出的暂扣或吊销相关证照的处罚。
3.如发生不依法报告或不立即组织抢救、擅离职守等严重行为的，法定代表人（实际控制人）接收上一年年收入 60%至 100%罚款，属于国家工作人员的，接受相应的党纪、政纪处分。
