□依据日本《电气用品安全法》第9条第1项第1号规定的模式(单独批次 型式实验)

□依据日本《电气用品安全法》第9条第1项第2号规定的模式（型式试验+工厂检查）

9．如依据日本《电气用品安全法》第9条第1项第2号规定的模式申请PSE认证，应将《工厂检查调查表》作为本申请书的附件一并提交。

10．申请产品的结构、材料及性能的概要，请填写第4页的表格随本申请书一并提交。

11．本次申请需要PSE证书副本份，测试报告副本（副本需要另付费）

12．本申请书所附加的资料如下（请在相应□内打）：
