第二炮兵总医院肝胆胃肠病研究所

中国人民解放军第二炮兵总医院是面向全国,为军内外患者提供医疗技术服务的综合性医院。医院始建于1954年5月,前身是北京军区第262医院,1999年9月28日,正式挂牌成立第二炮兵总医院。医院坐落于北京市西城区新街口外大街,距西单直线距离7公里,西邻北京师范大学、北京邮电大学,南邻积水潭医院,北邻北京大学医学部,位于北二环与北三环之间,紧邻地铁1号、2号、4号和10号线,交通十分便利。在中央领导、军委和第二炮兵领导的直接关怀下,经过10年多的开拓与发展,已建设成为一所技术先进、设备精良、专科齐全、人才优势、服务竭诚,集医疗、教学、科研、预防、保健和康复为一体的现代化程度比较高的大型综合性三级甲等医院,是第二、三、四军医大学临床教学医院,北京师范大学医学研究院,苏州大学第六附属医院,武汉大学医学院、辽宁锦州医学院和泰山医学院等院校研究生联合培养基地。

第二炮兵总医院肝胆胃肠病研究所成立于2007年6月,由普通外科、肝移植中心、肛肠外科、消化内科、中心实验室等多学科组成,组建了内窥镜中心和超声诊疗室,成立了专家会诊中心、消化病会诊中心,成为集临床、科研、教学等为一体的综合性肝胆胃肠病研究所。研究所现有在职人员48人其中教授/主任医师7人副教授/副主任医师

10人,博士生导师3人,硕士生导师5人,临床主系列均获硕士、博士学位。招收各类研究生10余人。另有包括4名院士在内的相关领域会诊专家10余人。研究所拥有一支高层次的护理队伍,精湛的技艺、热情的服务为各项新技术、新业务提供强有力的护理保障。形成了达芬奇机器人治疗肝胆胰等消化道恶性肿瘤、复杂胆道多次术后再次手术等高难手术,肝胆胰复杂疑难病和胃肠疾病等为特色的消化系统病诊疗体系,已打造出以机器人外科、肿瘤综合治疗等为特色的学科优势。

机器人外科处于国内领先国际先进水平。第二炮兵总医院于2008年12月在国内率先引进了达芬奇机器人手术系统,并在肝胆胰、心胸外科、泌尿外科、妇产科等领域,率先开展了机器人外科手术,创造了多项国内第一,并在肝胆胰机器人手术方面已处于国际领先水平。迄今已开展各类机器人外科手术近300余例,取得了宝贵的经验。周宁新教授以中国第一人成为世界临床机器人外科协会创始会员,出席临床机器人外科大会并担任共同主席,多次在国际机器人大会上作专题报告。为了进一步推动我国机器人外科的发展,增进相互交流和沟通,在国际临床机器人外科学会(CRSA)的授意下,研究所于2009年4月和2010年7月先后举办了两届达芬奇机器人手术演示及学术研讨会,邀请了国际著名机器人外科专家美国芝加哥伊力诺依大学Giulianotti教授,CRSA国际学术部主任、美国新泽西州Valley医院Anusak Yiengpruksawan教授,Francesco Bianco教授以及香港、韩国等教授进行手术演示与学术讲演,参会专家反响强烈,一致赞誉。2010年5月又成立了第二炮兵总医院达芬奇机器人微创外科临床基地、大陆机器人培训中心、机器人微创网站(www.daodoc.com)。根据机器人外科手术的成功经验,周宁新教授主编《机器人微创外科手术探索与实践》专著1部,重点阐述了肝胆胰、胃肠、泌尿、妇产、心胸等多学科领域中手术实践经验,反映当代微创外科的最先进技术。并发表相关论著和述评15余篇。目前,我国机器人外科发展已进入国际学术竞争的行列,为了更好地为我国的机器人外科人才团队提供一个发布最新国际学术动态、最新科研成果和展示不同学术观点的学术平台,2010年8月1日拟创办《机器人外科》杂志,旨在“全面快速报道全球机器人外科资讯,搭建中国机器人外科信息交流的平台”,以更好地推动我国机器人外科的快速发展。

研究所主要特色技术:肝胆胰疾病外科治疗居国内领先水平:肝门部胆管癌的诊治,经过30多年的临床积淀,居国内领先水平;复杂胆道多次术后重建属国内首创;完成复杂肝胆胰手术和肝移植术1500余例,无一手术死亡,技术领先,综合实力强,拥有在国内一流的肝移植团队和一整套特色康复治疗体系;胆管结石、肝胆胰肿瘤介入栓塞化疗等以微创治疗为主要特色。采用腹水超滤浓缩回输腹腔腹腔-颈内静脉转流消除顽固性肝硬化和癌性腹水;肝脏肿瘤引起阻塞性黄疸行“肝部分切除伴胰十二指肠联合切”等,临床效果显著,已形成一套完善的诊疗体系。在肝脏占位性病变,如原发性肝癌、转移性肝癌、肝血管瘤、肝囊肿以及肝脏外伤、肝硬化门脉高压、各种顽固性腹水等;肝内外胆道疾病,如胆囊结石、胆囊炎、胆道狭窄、胆管损伤、胆总管囊肿和胆道恶性肿瘤等;胰腺肿瘤(如胰腺癌和胰岛细胞瘤)及急慢性胰腺炎胰腺囊肿脾脏外伤肿瘤脾脏功能亢进等。肝脏、胰腺、胆道、脾脏疾病领域积累了丰富的临床经验。机器人治疗肝胆胰肿瘤居亚洲领先水平:机器人辅助下开展肝叶切除、肝门部胆管癌切除、胰头癌胰十二指肠切除、胰体尾切除、复杂胆道重建等腹部常见复杂手术,术后恢复快,无手术并发症,凸显机器人外科手术精细、微创、疑难等优势。胃、结直肠疾病诊治采取规范化标准化治疗方式处于国内先进水平:胃、结直肠恶性肿瘤采取规范化标准化根治术、中晚期胃结直肠癌术前动脉区域灌注化疗结合手术根治,明显提高5年生存率,该项技术处于国内先进水平。常见胃肠外科疾病,如肠梗阻、消化性溃疡、各类先天性胃肠畸形、肠瘘以及短肠综合征等,也积累了丰富的临床经验。甲状腺疾病外科治疗已形成微创化特色技术优势:主要是甲状腺和甲状旁腺良恶性肿瘤的外科治疗,如甲状腺癌规范化切除及淋巴结清扫,尤其是甲状腺良性肿瘤采用小切口腔镜辅助下甲状腺切除,切口小,微创,恢复快。在传统手术中常出现吞咽不适、肩背部僵硬、颈部麻木疼痛等症状,在微创手术中很少发生。甲状腺微小癌的早期诊断和治疗处于国内先进水平。肛肠疾病诊治独具特色:肛肠科是全军肛肠中心,主要开展包括肛门部常见病、肛门与大肠肿瘤、外伤、畸形、炎症性肠病以及顽固型便秘等临床诊疗,以及肛门部常见手术中与术后的局部疼痛问题,临床经验丰富。对顽固性便秘开展诊断与治疗,采用中西医结合治疗慢性溃疡性结肠炎等炎症性肠道疾病独具特色,开展了腔镜外科手术和内窥镜外科手术等微创手术,临床效果显著。乳腺疾病诊治经验丰富:通过B超、X线钼靶结合MRI,明显提高早期乳腺癌的发现率。对中、晚期乳腺癌,采取以改良根治为主,新辅助化疗、术后化疗、内分泌免疫治疗和中医治疗等为辅的全新综合治疗模式,效果显著,经验丰富。消化道肿瘤免疫、基因等综合治疗新模式

7.1 绿色治疗:应用海扶刀治疗了90余例肝肿瘤、胰腺肿瘤、乳腺肿瘤等手术,效果明显。7.2 免疫治疗:采用自体树突状细胞(Dendritic Cells,DC)和细胞因子诱导杀伤细胞(Cytokine Induced Killer cells,CIK)联合治疗姑息性术后肝胆胰肿瘤联合免疫治疗已形成特色,疗效显著。

7.3 射频治疗:在国际上率先开展了射频消融治疗肝硬化性脾功能亢进症和脾脏相关良性血液疾病等;采用无水酒精注射和射频消融治疗肝尾状叶、肝脏膈顶肿瘤及后腹膜转移癌等复杂部位肿瘤。

7.4 基因治疗:应用基因治疗中晚期恶性肿瘤,尤其是原发性肝癌及转移癌。7.5 靶向治疗:在肝胆胰中晚期肿瘤治疗方面,积累了丰富经验。

The General Hospital of PLA Second Artillery was built in May, 1954, with its former name the number 262 hospital of Beijing Military Area.It changed its name on September 28th, 1999 formally.The hospital locals at the Xin Jie Kou Wai Street of Xi Cheng District of Beijing, in between the North Second and third Ring Roads and is close to many subway stations.With more than ten years’ development, the general hospital of PLA second artillery has become a big and modern grade A tertiary general hospital, also the teaching hospital of the Second, Third and Fourth Military Medical University, and the base for cooperative education of graduate students for Beijing Normal University Medical Research Institute, the Sixth Affiliated Hospital of Suzhou University, Medical School of Wuhan, Jinzhou Medical University and Taishan Medical University.This 1200-bed hospital has 40 clinical and technical departments and many kinds of modern medical instruments with a total worth of 120 million yuan, which includes many of the world’s latest medical instrument such as Da Vinci Surgical Robot SI series, ELEKTA Precise medical linear accelerator, GE Healthcare's Light Speed CT750 HD, et al.The Institute of Hepatobiliary Gastrointestinal Disease of the General Hospital of PLA Second Artillery was founded in June, 2007.It consists of the many related departments including general surgery, anorectal surgery, digestive system medicine and ultrasound, also it consists of centers including liver transplantation center, lab center, endoscope center.Therefore it is a comprehensive famous for its special diagnosis and treating system for complex digestive system diseases with its advantages of robotic surgery and tumor comprehensive treatment.The robotic surgery of the institute ranks the first in China and is one of the world most advanced centers.Being the first hospital for installing the Da Vinci Surgical System in December, 2008, many kinds of Chinese first robotic surgery operations were performed at this hospital ever since, including many surgical fields like the hepatobiliary and pancreatic surgery, cardialthoracic surgery, urological surgery and institute, profeor Zhou Ningxin, has been giving lectures on robotic surgery many times at international conferences.Following the spirit of CRSA , profeor Zhou has held symposium on Da Vinci robotic surgery twice in Beijing, many world-top surgeons on robotic surgery such as Pier Cristoforo Giulianotti , Anusak Yiengpruksawan, Francesco Bianco and surgeons from Hongkong and Kerea were invited to hepatobiliary and pancreatic disease in China.We have a history of 30 years in treating hilar cholangiocarcinoma and have set many “ Chinese First” in biliary reconstructions after multiple complex biliary surgeries;we have a total number of 1500 operations of complex hepatobiliary and pancreatic surgery and liver transplantation, with zero perioperative mortality.Laparoscopic surgery and interventional therapy are also applied as minimally invasive techniques.Other techniques such as ultrafiltrated ascites intraperitoneal transfusion treating refractory ascites and partial hepatectomy combined with pancreatoduodenectomy in treating obstructive jaundice from hepatocarcinoma, et al were also widely applied.2.Robotic surgeries in hepatobiliary and pancreatic disease: hepatectomy, radical resection for hilar changiocarcinoma, pancreatoduodenectomy, body and tail pancreatectomy, complicated biliary reconstruction.3.Gastric and colorectal tumors , we use the standard treatments including preoperative regional intraarteral chemotherapy.For thyroid benign tumors, we use laparoscopic aisted thyroidectomy with minimal incision.4.Department of anorectal surgery is the center for anorectal disease of PLA, most of the common anorectal diseases are diagnosed and treated at this center, including the combined treatment of traditional Chinese medicine and western medicine for obstipation and chronic ulcerative colitis.Laparoscopic and endoscopic surgeries are also carried out.5.New combined treatment for digestive system tumors with tumor immunology and gene therapy.Many new techniques like high intensive focused ultrasound, immuno cell therapy using dendritic cells and cytokine induced killer cells,radio frequency, gene therapy and targeted therapy.The General
