受伤程度：左腿2处骨折，左脚踝2处骨折（伤残鉴定：预计为9级）
Injured degree: two fractures locate in her left leg and another two in her left ankle.(Disability identification: expected for level 9)
事故经过course of events ：
由于离心机滤液沉淀池沉淀的氢钙没有及时回收（因为液下泵不好使），导致池内充满氢钙，又开始组织干燥工清理控池内氢钙2天。这时氢钙堆积2米多高，已将无轴螺旋输送机底半部埋入1米多高。
Because the sedimentary dicalcium phosphate from the centrifuge filter pit was not recycled in time.(submerged pump didn’t work well), the pit was fulfilled with dicalcium phosphate.It takes two days to organize drying workers to clean the pit.At that time it piled up more than two meters, the bottom of the shaftle screw conveyer was embedded more than one meter.这时开始组织人员开始人工上料烘干，白班人工上料，中途阻风卸料阀坏检2次，到晚上8点20分左右修好，又开始安排人工传递上料，并把怎样上料告诉安排给当班班长，但我们走后，他们私自使用已停用的无轴螺旋输送机上料。2011年4月5日晚21时许，当班离心机工于海华在设备有异常响动时下来观察设备底部情况，由于氢钙堆积1米多高，她是站在这个上面观察的，而无轴螺旋输送机在其中埋着半截，她认为无轴螺旋输送机几天都没有开动，没有注意，返回时左脚踏在五轴螺旋上，导致左腿和左踝骨骨折，需要治疗疗养12个月。
Then we organize workers start to dry through manual feeding in the day time, the resistance wind unloading valve didn’t work two times halfway。After it got repaired at 8:20 pm, we start to arrange manual feeding again and tell the duty monitor how it went.But after our leaving，they use the idle shaftle screw conveyer privately to feed.At 9:00 pm on 5th April 2011, the duty centrifuge operator Yu Haihua stood on the dicalcium phosphate pile which is over one meter high to watch the bottom of the centrifuge because the equipment made unusual noises.Because half of the conveyer was covered and Yu Haihua thought that the conveyer had turned off for several days, she paid little attention on her way back to the platform and stepped on the 5 axis spiral.As a result, her left leg and left ankle got fractured.12 months will be needed to treat and recuperate.整改意见/措施 Action to improve：
1、由于无轴螺旋输送机在试车上料时发现不好使。氢钙湿粉不断把机器中的螺旋抬高，而湿粉垫入U型槽底部，且把湿粉挤压成湿饼。进入干燥器中把干燥器进料口上端堵塞，所以于3月末即停止使用了，并解决干燥剂堵料问题用了三天时间，使离心机下来的氢钙堆积，改为暂时人工传递上料，准备买一台皮带机更换取代。
1、When feed material in the trial run, we find the shaftle screw conveyer didn’t work well.The wet dicalcium phosphate powder raised the spiral again and again.Meanwhile the wet powder was extruded into wet cake at the bottom of the U tank.After entering the dryer the feed inlet of the dryer got jammed and we didn’t use it since the end of March.We spent three days to clean the dryer jam and the dicalcium phosphate piled up.We just changed into manual feeding material temporarily, and we plan to buy a belt conveyer and use it instead.HR/ Admin 7th April 2011
